Click on the following links to download the various waiver release forms, which must be signed if you are participating in a CMRC sanctioned event. If you are under 18 years of age it is recommended that you print the waivers at home and have a parent sign the Parental Waiver and witness the Minor Participant Waiver and bring them to the track with you. No form means no racing!
Race Entry & Waiver
This form must be completed in order to sign in for a race. You simply fill in the information required and sign where indicated. If you are not sure of which class to sign up for you can leave this section until you arrive at sign in. For more information about which classes to sign up for go to Member Info where you will find the classes and their respective age and quad requirements. If you are under 18 years old you and your parent must sign this form on both pages where indicated. We must have this form signed by a parent in order for you to race. If you are attending a CMRC ATV event there are two days of racing which require 2 race entry forms. Be sure to have a parent sign both forms.
Parental Consent and Minor Participant Waiver
These waivers must be completed in full once a year prior to your first race. A parent of a minor racer (anyone under 18 yrs) must complete the Parental Consent form, sign and print their name at the bottom. The Minor Participant Waiver must be completed by the minor. He/she must enter their age in the appropriate space and sign and print their name at the bottom of the form. The parent can sign as the witness. Bring these with you to your first race and present them with your completed race entry form. We will keep these forms on file and you will receive a card that indicates you and your parent(s) have signed the necessary forms. You can present this card at any CMRC sanctioned event for 2010 without having to complete these forms again. You will, however, be required to complete a race entry form each and every time you race with your parent(s) signature.
